Tree order forms can be submit using our online order form or with our PDF form, both will be available below next year for the 2025 sale. Paper copies can be picked up at the USDA Service Center (1209 Bear Lane, Monticello IL). If you would like a form mailed to you, please call (217) 762-2571 ext. 101
